Certified Nursing Assistant 

Position Overview 

Responsible for providing routine daily nursing services to assigned residents in a manner that is caring, compassionate  and customer-oriented and in accordance with laws, regulations and facility guidelines.  

Basic Qualifications 

Education & Certification: 

High school diploma or equivalent; successfully completed a state-approved Certified Nursing Assistance program; must  possess a current certification in good standing, or be able to obtain certification within four months of employment, if  allowable by the state; must have CPR certification. 

Skills & Requirements: 

Must possess the ability to read, write and speak the English language, communicate resident condition to others and  make independent decisions as needed; must have common working knowledge of all rules, laws and regulations as  defined by the state as it relates to skilled nursing; must be patient, friendly, courteous and respectful to residents in any  situation, including difficult ones; must be able to move frequently throughout the workday, answer resident call lights,  assist with baths, showers, toileting, feeding and personal grooming of the residents; must be able to handle sensitive  situations related to the emotional well-being of the residents with patience, dignity and respect; must be able to lift 40 lbs  floor to waist; lift 10 lbs waist to shoulder; lift and carry 40 lbs; and push/pull 40 lbs. 

Experience: 

Minimum of six months experience in a long-term care or other geriatric care environment is preferred. 

Working Conditions 

Works specifically in the nursing department, resident rooms, shower rooms, etc.; bends, lifts, sits, stands and moves  frequently throughout the shift; deals with frequent interruptions and possibly upset residents and family members; works  regularly scheduled shifts with possible weekends, holidays and other hours based on resident needs; primary  responsibility is care of residents but also deals with visitors, government personnel, etc.; participates in required  continuing education programs; subject to odors, falls, burns and exposure to infectious diseases, waste, etc., including  AIDS and Hepatitis B viruses.

Resident Rights: Promptly reports resident grievances to the nursing supervisors; treats all resident care information  with confidentiality; continuously monitors assigned areas to ensure residents are treated fairly and with dignity and  respect at all times; conducts nursing care in privacy, including knocking on the door prior to entering a resident’s room. 

Safety Awareness: Follows safety program guidelines; immediately reports accidents and incidents to supervisors;  identifies unsafe working areas and promptly reports to supervisors; promotes safety by working as safely and efficiently  as possible; monitors resident rooms to ensure it is clean and neat at all times; consistently follows infection control and  universal precautions and other guidelines; identifies and corrects or reports hazardous conditions to supervisors;  understands facility emergency and evacuation protocols. 

Organizational Skills: Completes assignments in an organized fashion by the end of scheduled shifts; operates with  flexibility and shows attention to detail when recording information in resident charts. 

Customer Service: Treats all residents, families, co-workers and supervisors with dignity and respect and takes initiative  to exceed customer expectations; professional represents the facility, self and position to visitors and residents;  demonstrates a positive attitude and willingness to help to all visitors and residents; remains sensitive to all needs through  listening, observing and responding appropriately for the position held.

Essential Job Duties 

1. Provides quality nursing care to residents and communicates to each resident and family member to build positive  relations. 

2. Routinely makes rounds and responsively provides proper resident care and service. 

3. Assesses residents for condition changes and reports to nurse supervisor. 

4. Answers resident call lights promptly and courteously. 

5. Delivers meals and performs feeding assistance to residents and provides hydration. 

6. Performs vital sign monitoring/charting and observes residents for changes in condition or behavior and reports to  nursing supervisors. 

7. Provides assistance to residents for showers or baths. 

8. Provides assistance with lifting, turning, moving, transporting or transferring residents. 9. Monitors incontinent residents to ensure bed linens and clothing are clean and dry. 

10. Assists residents with toileting and bed pans and collects urine, stool and sputum samples as directed. 11. Ensures good hygiene through oral care, hair care, shaving, etc. 

12. Changes bed linens and makes beds. 

13. Provides proper skin care and promptly reports observations of skin breakdown and pressure areas. 14. Communicates changes and follows plan of care effectively by recording changes in charts. 

Other Job Duties 

1. Adheres to facility admission, discharge and transfer policies and prepares resident rooms for arrival. 2. Labels, organizes and stores resident’s personal belongings, clothing, etc. 

3. Provides support and assistance for dying residents and post-mortem care. 

4. Receives report from off-going shift or supervisor to better understand resident needs and changes. 5. Promotes interdepartmental teamwork to meet resident needs. 

6. Promotes an environment of positive public relations by displaying a calm, caring, warm and friendly atmosphere  in department and on each shift. 

7. Keeps residents’ water pitchers clean and filled with fresh was and within reach of the resident. 8. Provides training to new CNAs, upon request. 

9. Ensures cleanliness of water pitchers and provides refills of fresh water as needed. 

10. Transfer residents to and from dining room, activities, therapy, etc. 

11. Works well with supervision, team members and other departments to properly care for each resident. 12. Rotates residents on a two-hour schedule. 

13. Monitors dressings, bandages, etc. and changes, as directed. 

14. Arrives to work as scheduled and makes rounds with previous shift. 

15. As instructed and appropriate, collects specimens (urine, stools, etc.).

LPN / LVN 

Position Overview 

Provides care to residents in accordance with care plan as directed by physician’s orders, manages and directs nursing  employees assigned to unit and follows the direction of nursing administration in accordance with laws, regulations and  facility guidelines. 

Basic Qualifications 

Education and Certification: 

LPN or LVN degree from an accredited Nursing School; current licensure within the state of practice; CPR certification  required. 

Skills & Requirements: 

Must possess the ability to read, write and speak the English language; must be involved in health care matters and  projects with the community as needed; must have strong communication and presentation skills in both group and  individual situations; must be patient, friendly, courteous and respectful to residents in any situation, including difficult  ones; must be able to make independent decisions when necessary; must have the ability to follow nursing department guidelines as outlined by the facility and other regulations governing nursing services in a long-term care facility; must be  able to move frequently throughout the workday; must be able to lift 40 lbs floor to waist; lift 20 lbs waist to shoulder; lift  and carry 40 lbs; and push/pull 40 lbs. 

Experience: 

Minimum of one year nursing experience in healthcare setting; geriatrics experience preferred. Working Conditions

Essential Job Duties 

1. Successfully delivers basic nursing care to residents under in accordance with care plans as ordered by a  physician. 

2. Conducts ongoing assessments of residents to identify care needs and makes needed revisions to the care plan  based on changes in resident condition. 

3. Effectively meets all care needs of the residents through timely completion of assigned duties. 4. Responsible for a wide variety of nursing administrative duties. 

5. Conducts rounds within the nursing department to ensure quality care of residents. 

6. Accurately documents nursing care notes in resident charts in a timely manner. 

7. Performs medication administration duties in accordance with the care plan as ordered by a physician. 

8. Monitors progress of the residents and makes suggestions regarding treatment plans which may further meet their  care needs. 

9. Observes resident behavior, vital signs, symptoms, responses to treatment and reports findings to nursing  supervisors. 

10. Documents interventions and resident or family education efforts timely and accurately. 

11. Completes resident incident reports upon identifying changes in the condition of residents. 12. Monitors residents’ skin to identify the risk of pressure sores and promptly reports to the nursing supervisors. 13. Places medication and refill orders with the resident pharmacies as needed. 

14. Monitors the emotional and physical conditions of the residents to ensure comfort and safety. 15. Performs shift change reports with the previous shift nurses and ensures proper follow up. 16. Understands and follows proper procedures for admission, transfer and discharge of the residents. 17. Provides expectations and monitors performance of specific nursing staff. 

18. Directs and develops the CNAs to ensure the delivery of high quality care and services in accordance with all  laws, regulations and facility guidelines. 

19. Delegates assignments and provides overall supervision to CNAs. 

20. Effectively communicates guidelines and procedures to CNAs, Restorative Aides, residents, families and  physicians.

21. Provides overall supervision for timely responses to call lights; responds personally as needed. 

22. Assists in providing on-the-job training for new CNAs and LPNs. 

23. Assists in promoting a team environment through effectively communicating praise and recognition with the  CNAs. 

24. Monitors schedule adherence of staff for lunch and rest breaks, beginning and ending of shifts and overtime. 25. Monitors job performance of CNAs and provides correction as needed 

Other Job Duties 

26. May participate in development and delivery of in-service training programs. 

27. Participates in department orientation program and provides ongoing support to new employees. 28. Assists with communicating new admissions with nursing staff to ensure rooms are prepared for arrival. 29. Provides direct nursing care as necessary. 

30. Promotes continuity of care through partnering with other departments (i.e. Rehab). 

31. Participates in discharge planning process. 

32. Actively participates in state or federal survey processes in conjunction with nursing administration. 33. Assists in educating CNAs on facility guidelines regarding resident right to refuse treatments.
